We are thrilled to announce the return of the It Takes a Village Wellness Day!
In partnership with the CSULB Nursing program, we have been providing supplemental health care coordination and case management out of the CityHeART Resource Hub for more than a year.
CSULB Nursing students in their final class — COMMUNITY HEALTH — spend a semester with us at the Villages at Cabrillo providing supplemental health care case management, care coordination, and drop-in wellness screenings and meaningful education for Veterans, seniors, and families we serve.
This partnership began during the COVID-19 pandemic, and these students have provided COVID-19 related education AND vaccines to the residents at the Villages as part of this program.
These students run a weekly wellness day for drop-in access to support services such as medication management, blood pressure and BMI screenings, and glucose readings.
They work in pairs to provide supplemental case management to individuals over the course of their semester, and they meet weekly with their clients and provide progress reports to housing case managers as part of the care team — which is what the Resource Hub is all about, extending the bandwidth of a case manager and building their client’s network of support alongside the care team.
The students also each facilitate a pair of educational workshops related to topics such as stress management, diet & nutrition, & healthy coping mechanisms.
Building off the previous semester’s progress, each cohort of students completes their course by providing a presentation to the community at the Villages at Cabrillo about health and wellness needs, including recommendations for next steps.
In their first two weeks with us, so far they have already supported about 10 individuals with health and wellness needs AND they have implemented a meaningful collaboration with the Century Villages at Cabrillo Oasis Residential Services Occupational Therapy program to work together on increasing physical activity of the community at the Villages.

Funding for this program is provided by Dignity Health through CVC”s Pathways to Health program.
